All registered students in the College of Arts and Sciences (CAS) are required to have health insurance coverage. All registered students taking 6 or more credits and students in Business and Graduate Studies (BGS), School of Education (EDU), School of Professional Studies (SPS), and School of Nursing and Health Professionals (except those who began as CAS students) are eligible to enroll in this Student Health Insurance Plan on a voluntary basis.

Eligible Students who DO NOT WANT to be enrolled in the Fall Student Health Insurance Plan must submit an online Waiver Form. Recognizing that health insurance situations may change, students will be required to provide proof of comparable coverage each academic year in order to waive participation in the Student Health Insurance Plan. **WHEN COMPLETING YOUR WAIVER AN UPLOAD OF YOUR INSURANCE ID CARD IS NOT REQUIRED.**

Our student health insurance department will review your waiver request and send approval or denial to your email address within 30 days.
